# What documents do I need for the online application package?

**The following documents will need to be uploaded in PDF or a photo in .jpg format:**

- Birth certificate or passport for each student.
- The most recent report card for each student.
- The student’s most recent IEP and IEP report, LAP and LAP report (if applicable)
- Any report(s) from the Child Development Centre, Ministry of Children and Family Development, hospital and/or health agencies (if applicable)
- Relevant legal guardian documents (if applicable)
- Relevant legal custody documents (if applicable)
- Relevant immigration or citizenship documents (if applicable)
- Proof of residency
